DOT physicals for CDL holders - $90 ($80 for active duty)

Complete DOT physicals for CDL holders include eye exam, hearing check, vital signs, kidney function check via urinalysis, medical history review, and physical exam. All exams are completed and results determined based on DOT requirements and guidelines. 3bExam software is utilized for exam completion. A link will be sent to you prior to your appointment allowing you to complete required information prior to your visit. We are happy to assist you with these forms if needed. Exams are by appointment to offer faster service as we value your time.

Merchant Mariners - Coast Guard exam (form CG-719K) $100

The paper form CG-719K must be brought to this exam with health history completed. The history will be reviewed and exam completed including eye and hearing assessment, vitals and head to toe health assessment exam. All exams are completed and results determined based on DOT requirements and guidelines. If a urine drug screen is required, there is an additional $30 cost. All paperwork is completed at the time of exam.

Pre-employment physicals (basic $50)

Pre-employment physicals are completed as directed by the hiring entity via their paperwork. Please complete all medical history portions of forms prior to appointment. This history will be reviewed with you during your exam. Depending on the type of work that you are being hired for, you may be required to demonstrate ability to squat, bend, lift specific weight and/or jog. Please wear comfortable clothing that will allow you to demonstrate these movements freely. Your hiring company may also require you to submit a sample for a urine drug screen. The result of this screening is sent directly from the lab to your hiring employer.

 

Sexual Assault Exams - No charge to you

Sexual Assault Exams are conducted by a certified Sexual Assault Nurse Examiner. These exams are paid for by the State of NC. They are offered to:

  • Male and Female

  • Ages 16 years and older (age 16 & 17 year olds must have consent from parent or guardian)

  • Exams must be completed within 120 hours after assault (5 days)

School Physicals - $40

School physicals are completed based on the exam requirements outlined by the paperwork your child’s school provides. Please bring this paperwork with you to your appointment with health history portion completed. Please also bring a copy of the child’s immunization record.

 

 Sports Physicals - $40

Sports physicals are completed based on the indicated requirements of the athletic department, as noted on their provided paperwork, and to establish that the patient is able to safely participate in the indicated sport. Please bring the paperwork provided by the school/program to your exam with the medial history portion completed.

 

AHA BLS for Providers Class - CPR

CPR:

American Heart Association BLS for Providers class offered weekly. $85 paid at time of class.

Skills check-off for blended AHA BLS for Providers learning by appointment ($55) Bring a copy of your online class completion certificate.
